To complete the reenrollment process, you will be asked to provide the following information:

- Updated Immunization Records
- Authorization to Disclose Immunization Information and or Revocation Form
- Medical Forms A&B
- Physical Examination Form, required for new students
- Updated Court/Custody Documents, if applicable
- Information needed to make an online payment for enrollment fee
- EdChoice Scholarship Request/Renewal Form
- Student is new to the EdChoice Program- EdChoice Scholarship Program Request Form 2024-2025
- Student is a returning EdChoice Student- EdChoice Scholarship Program Renewal Form 2024-2025
If participating in the EdChoice Scholarship Program, Proof of Address is required.
After you have uploaded all documents and completed the enrollment packet, a Submit Packet and Make Payment form will appear. Please follow the instructions that will be provided to submit the enrollment packet along with the non-refundable enrollment fee as follows:

To complete the application process, you will be asked to provide the following information:

- Up-to-date Custody Verification Papers Included (if applicable)
- Completed Adoption Papers Included (if applicable)
- Copy of ETR, IEP or 504 (if applicable)
- Copy of Official Stamped Birth Certificate (all 4 corners of the certificate must be visible)
- Information to make online payment for application fee

Once your application is accepted, you will need to create and sign in to the Facts Mgt Family Portal to enroll your student(s).
